Metformin for PCOS (now called PMOS): what it does, side effects and real cost

Metformin is one of the most prescribed medicines for PMOS, formerly PCOS, in the Philippines. It is cheap, well studied, and often misunderstood. Here is what it does, how to handle the stomach upset, and why the generic is the same medicine as the brand.

Editorially reviewed on October 29, 2026. [TODO: medical reviewer] How we review

Quick answer

Metformin helps your body respond better to insulin. The 2023 International Guideline says it should be considered in adults with PCOS and a BMI of 25 or higher (Rec 4.3.1), and that it is preferred when the goal is metabolic health (Rec 4.4.2).

There is no “PCOS metformin.” Generic metformin 500 mg costs about ₱2 per tablet to ₱3.75 per tablet, versus ₱18.25 per tablet for Glucophage. Same molecule. Stomach upset is common early; extended-release forms often help. PhilHealth Konsulta lists metformin among its medicines.

Myth: “May metformin ba na para sa PCOS talaga?”

A Filipina on r/PCOSPhilippines asked whether a metformin sold by her OB's receptionist was “specific for PCOS” (thread). It was not. Every metformin tablet contains the same active ingredient, metformin hydrochloride, whatever the box says. Generics, RiteMed and Glucophage differ in price and in whether they are immediate or extended release, not in being made for PCOS.

That thread points to another rule worth knowing: in the Philippines, dispensing is reserved for pharmacists, and doctors and clinics may not sell medicines without FDA authorisation. You can fill your prescription at any licensed pharmacy, including for a cheaper generic if your doctor allows it.

How it works

Many women with PCOS have insulin resistance: the body needs more insulin than usual to keep blood sugar steady, and high insulin pushes the ovaries to make more androgens. Metformin lowers the liver's sugar output and helps muscles use insulin. Over months, that can support more regular cycles and better blood sugar and cholesterol results. It is not a weight-loss drug, and it works the same whatever your size. It does not change who you are or how hard you try; insulin resistance is biology, not willpower.

What the 2023 Guideline says

  • Rec 4.3.1: metformin should be considered in adults with PCOS and a BMI of 25 or higher.
  • Rec 4.4.2: where the goal is metabolic (blood sugar, cholesterol), metformin is preferred over the pill.
  • Metformin can be used with or without a combined pill, depending on your goals. For fertility it is one option alongside clomiphene (see the fertility guide).

Whatever you take, the guideline still asks for a 75 g OGTT at diagnosis and every 1 to 3 years, plus a lipid profile and yearly blood pressure. Metformin does not replace those checks.

Who it suits

  • Women with signs of insulin resistance, borderline blood sugar, or a family history of diabetes
  • Those whose main goal is metabolic health
  • Some women who cannot take a combined pill, or who take one alongside

Weight is one factor in the guideline, not a judgment. If a doctor has made you feel that PCOS is your fault, that is weight stigma, and the guideline asks clinicians to recognise it (Rec 3.6.1, 3.6.2). You deserve a doctor who explains options calmly. Our doctor directory lists community-recommended OB-GYNs and endocrinologists.

Stomach side effects and what helps

Nausea, loose stools, cramps and a metallic taste are common in the first weeks. In a Philippine study of women with PCOS, one participant said she “completely discontinued it” because of vomiting (Garcia et al. 2025). You do not have to suffer in silence. Things doctors commonly do:

  • Start with a low amount and build up slowly
  • Take it with or right after meals
  • Switch to an extended-release (XR or SR) form, often easier on the stomach. Metformin SR costs about ₱6.25 per tablet (about ₱375 a month in the same cost example)

Tell your doctor before stopping. Many people who quit early would have done fine with a slower start or an XR tablet.

“May metformin ba na para sa PCOS talaga?”

Metformin upsets my stomach. Is there a PCOS-specific one?

There is no PCOS-specific metformin; it is the same medicine whatever the box says, and generics cost far less than the brand. Stomach upset is common early. Doctors often start low, ask you to take it with meals, or switch to an extended-release form.

“Borderline diabetic na raw ako.”

I'm borderline diabetic.

PCOS raises the risk of type 2 diabetes and heart disease, so the guideline asks for an OGTT at diagnosis and every 1 to 3 years, plus lipids and blood pressure. In a Filipino sample, the irregular-cycle phenotypes had more metabolic problems.

“Ang hirap pumayat kahit anong gawin ko.”

I can't lose weight no matter what I do.

Insulin resistance makes this genuinely harder; it is not a willpower problem. Healthy eating and movement help everyone with PCOS at any weight, and no single diet is best (Rec 3.1.1, 3.3.1). Doctors are also asked to screen for eating disorders regardless of weight (2.5.1).

“Nagagamot ba ang PCOS? PCOS-free na ba ako?”

Is PCOS curable? Can I become PCOS-free?

There is no cure, but symptoms can be managed well. A normal ultrasound later does not remove metabolic risk, so keep doing your checks (OGTT every 1 to 3 years). Stopping care because it is 'incurable' is one of the main reasons Filipinas drop out of care.

Generic vs Glucophage: what it costs

Generic metformin is one of the cheapest medicines in PCOS care. At the brand price, the same month can cost several times more. PhilHealth Konsulta lists metformin among its benefits “as needed as part of case management”, so ask your Konsulta provider. See the full cost guide.

Safety

Tell your doctor about kidney or liver problems

Metformin is cleared by the kidneys. Doctors check kidney function before and during treatment, and adjust or stop it if needed. [TODO: source: metformin product label]

Before scans with contrast dye or surgery

Tell the radiology team or surgeon you take metformin. They may ask you to pause it for a short time. [TODO: source: metformin product label]

Pregnancy and trying to conceive

Some women stay on metformin while trying to conceive; others stop. Ask your doctor what fits your plan.

Taking inositol too?

Many Filipinas take inositol alongside metformin. Tell your doctor about every supplement. See our inositol guide.

Frequently asked questions

May metformin ba na para sa PCOS lang?

No. All metformin contains the same active ingredient. Generic and brand versions differ in price and release type (immediate or extended), not in being made for PCOS.

How long does metformin take to work for PCOS?

Usually about 3 to 6 months for effects on cycles and metabolic results. Stomach side effects tend to ease in the first weeks.

Is metformin only for overweight women with PCOS?

The guideline says it should be considered in adults with a BMI of 25 or higher, and it is preferred for metabolic goals. Doctors may also use it in other situations; ask what fits you.

Generic or Glucophage: same ba?

Same molecule. Generic 500 mg tablets cost a fraction of Glucophage. Ask your doctor if a generic is fine for you.

Does PhilHealth cover metformin?

PhilHealth Konsulta lists metformin among its medicines as needed as part of case management. Availability depends on your Konsulta provider.

Can I take metformin and the pill together?

Yes, doctors often combine them, depending on your goals. Ask your doctor.

Is PMOS the same as PCOS?

Yes. PMOS (polyendocrine metabolic ovarian syndrome) is the new name for PCOS, announced in The Lancet on 12 May 2026. It is the same condition with the same diagnosis and the same treatments. Both names will be used until the 2028 International Guideline update.

Not the only option

Healthy eating and movement help everyone with PCOS at any weight, and no single diet is best (Rec 3.1.1, 3.3.1). These work alongside metformin, not as a test you have to pass first.
